Our Centralized Intake team is moving into a larger office space to better serve you. Because of this move, the main Centralized Intake and Centralized Scheduling phone numbers and fax line will be temporarily unavailable on Tuesday, December 2, from 12:30 to 4:00 p.m.
During this time, phone calls will go to voicemail. If you call during this window, please leave a message with your name, phone number, and a brief reason for your call.
