We are the local Community Developmental Disabilities Program, or CDDP. We serve children and adults with intellectual and developmental disabilities.
An intellectual or developmental disability, also called I/DD, is a lifelong condition that can affect how a person learns, communicates, or cares for themselves.
We work under a contract with the Oregon Department of Human Services and the Office of Developmental Disabilities Services. These state agencies set the rules for the services we provide.
Our goal is to help people get the supports they need to live, work, and take part in their community.
Services We Provide
We may provide or arrange the services listed below.
Service Coordination and Case Management
We help individuals and families:
- Understand available services
- Set goals and make a service plan
- Connect with providers
- Review services to make sure they are working
Case management means helping plan and organize services to meet a person's needs.
Intake and Eligibility
We:
- Take referrals and applications
- Review records and assessments
- Decide if someone qualifies for I/DD services under state rules
Eligibility means meeting the state's requirements to receive services.
Family Support
We support families who care for a child or adult with I/DD. This may include helping them find resources or apply for available funding.
In-Home Services
We help arrange services that support people in their own homes. These services help individuals stay as independent as possible.
Residential Services
We help people find residential programs if they cannot live on their own. These programs provide support and supervision in licensed homes or facilities.
Employment Services for Adults
We help adults prepare for, find, and keep jobs in the community.
Transportation Services for Adults
We may help arrange transportation related to approved services or employment supports.
Referral to Brokerages for Adults
Some adults receive services through a brokerage. A brokerage is a community agency that helps adults manage their own support services.
We can provide referrals and help with the transition to brokerage services when needed.
Planning and Provider Oversight
We:
- Develop services to meet community needs
- Contract with service providers
- Monitor providers to make sure they follow state rules
