SUMMARY OF SERVICES
Our disease prevention team works to advance policies, environments, and systems that promote health and prevent, reduce, and manage chronic disease.
The Centers for Disease Control (CDC) defines chronic diseases as conditions that last one year or more and require ongoing medical attention or limit activities of daily living or both. Common chronic diseases include:
- Arthritis
- Asthma
- Cancer
- Chronic Obstructive Pulmonary Disease (COPD)
- Diabetes
- Osteoporosis
- Select heart, stroke and vascular diseases

Chronic Disease Self-Management Programs:

Center 50+ offers a full variety of in-person and virtual classes, activities, and services. This includes programming directly related to chronic disease prevention and management, as well as general wellness and entertainment activities.
- Salem Health's Community Health Education Center offers classes, support groups, and a library of resources for community members experiencing chronic disease.
- The YMCA of Marion and Polk Counties offers several programs designed for chronic disease prevention and management. These programs include: Aging Together, Pedaling for Parkinson's, Livestrong®, Diabetes Prevention Program, and Healthy Weight and Your Child.
