What is a CHA



The Community Health Assessment (CHA) is a region specific health assessment that identifies key health needs through systematic, comprehensive data collection and analysis. The CHA consists of various data sources (such as vital statistics, surveys, etc.) that provide important information of improving our community's health.  The CHA captures the community's unique insight and perspective to provide a deeper understanding of the issues Marion and Polk County residents feel are important.

Marion-Polk Community Health Collaborative




The Marion-Polk Community Health Collaborative is comprised of community partners in Marion and Polk Counties who work together to collectively improve health issues that have been identified by the community. The Collaborative is comprised of an Executive Committee, Steering Committee, and various subcommittees and workgroups who strive to help our community achieve their best health and well-being.
