How to Submit Your Complaint
If you are not getting the services you believe you should receive, or a decision has been made about your services that you do not think is the right decision for you, you have the right to complain. To submit a complaint, please complete the IDD Services Complaint Form. Completed forms may be mailed to our office (address above), faxed to (503) 588-5290, or emailed by clicking HERE.
