Getting Ready for Your Appointment
Beginning August 10, the check-in process for your WIC your appointment will look a bit different! You will receive a text reminder a couple of days before your appointment with a link to complete a digital check-in. This allows us to collect all of the information required for your appointment ahead of time to ensure we will be able to complete your appointment and issue food benefits per USDA requirements. During the digital check-in process you will be able to upload photos, documents, and files for proofs and measurements, provide information required for enrollment, and select if you would like to have an in-person appointment or one completed by video. After you have submitted your digital check-in, staff will update your record and send you a text notice that your check-in is complete.
Required Proofs
Each time you reapply for WIC services, you must show proof of identification, income, and residence.
Please look through the following information on what we can accept for proofs to prepare for your WIC appointment. If you have question about other types of proofs WIC can accept, please call, text, or email us before your appointment so we can explore options that may work for you..
Please provide one for yourself:
- Photo ID ( drivers license, state ID, passport)
- Work or school ID
- OHP medical card
- Pay stubs
- Voters registration card
- Birth record
Please provide one item for each child with an appointment:
- Birth certificate or birth record
- Photo ID (passport, state ID)
- OHP medical card
- Immunization record
- Social security card
Please provide items for each income coming into the household:
- OHP enrollment letter
- SNAP award letter
- TANF award letter
- Pay stubs
- SSI or SSD
- Most recent W2 or tax return
- Foster child/parent placement letter
- Signed letter form your employer stating gross income
Please provide one item that shows the address where you live and your mailing address if you use one:
- Oregon drivers license or ID card
- Current bill
- Mail or letter addressed to you
- Bank statement
- Rent receipt
Weight, Height, and Iron Information
We also need current weight, height, and hemoglobin/Hematocrit results (bloodwork that looks at iron levels) for each person with an appointment. Take a look at the options below to find one that works for your family:
If you are planning on coming in to the office for an in-person appointment, please bring everyone who has a scheduled appointment and we will complete the medical data checks for everyone present during this time.
You can provide us with weights, heights, and hemoglobin/hematocrit (bloodwork) results from a health or medical provider visit, nurse home visit, or Head Start health screening during digital check-in.
- Measurements need to have been collected within 60 days of your WIC appointment.
- For infants 2 months and younger, birth measurements can be used.
- We can no longer use measurements completed by parents or caregivers
