Summary of Services
The Housing Programs and Services mission is to assist individuals and families facing serious and persistent mental illness, substance use disorders, and/or intellectual/developmental disabilities. We strive to help them find, afford, and maintain long-term, safe, and secure housing by providing:
- Housing navigation
- Housing subsidy
- Case management and care coordination
- Peer support services

Horizon House is our residential treatment facility that offers high quality, evidence-based mental health treatment options in a compassionate, trauma-informed, patient centered manner. Horizon House residents are supported to achieve their highest level of health so that they can transition to the lowest level of care and move toward independent living.
Services offered at Horizon House include:
- Case management
- Individual and group skills training
- Group therapies
- Medication management
- Budgeting and money management skills training
- Residential treatment for intensive 24-hour help
- Weekly community outings (i.e., grocery store, gym, community-based support meetings)

Human Services Housing Referrals
Referrals require a completed referral form and a mental health assessment (given within the past 12 months). If you need assistance with the referral form or with an assessment, our team can connect you to someone that can help. Fax completed forms to 503-588-5367 - ATTN: Rental Assistance Programs. Completed referral forms may also be emailed to: HHS-Housing@co.marion.or.us.
